Immunotherapy for childhood aplastic anemia in India: a case for universal healthcare?
نویسنده
چکیده
Aplastic anemia in childhood is a difficult condition to treat, and Nair and colleagues are to be commended for their meticulous collection of single-center data over a 22year period [1]. They evaluated the efficacy of immunosuppressive therapy (IST) with antithymocyte globulin (ATG) and cyclosporine (CSA) on 33 pediatric patients with acquired aplastic anemia in the Indian setting. Their results indicate that in India, with appropriate supportive care, pediatric aplastic anemia patients treated with IST can achieve response rates similar to North America and Europe, where the response rate is as high as 81% [2]. In addition, they confirm the experience of other authors, that ATG and CSA alone are the backbone of IST in treating childhood aplastic anemia, and that administration of G-CSF reduced infectious complications, but had no impact on survival or outcome [3].

Although aplastic anemia (AA) and myelodysplastic syndrome (MDS) are separate entities with different management, distinction between the two can be difficult on morphological basis due to hypocellularity of bone marrow. MDS is one of the serious complications of AA. Karyotyping is definitive in the diagnosis of MDS. Better and robust investigations like 18F-Fluoro-deoxy-Glucose Posi...
متن کاملFalse Negativity of Tc-99m Labeled Sodium Phytate Bone Marrow Imaging Under the Effect of G-CSF Prescription in Aplastic Anemia: A Case Report
Granulocyte colony-stimulating factor (G-CSF) is a hematopoietic cytokine which controls the differentiation and growth of hematopoietic cells in the bone marrow. We report a severe aplastic anemia (SAA) patient with false-negative 99mTc sodium phytate bone marrow imaging findings under concurrent G-CSF therapy. The first bone marrow imaging showed a normal bone marrow activity. However, the bo...

Human parvovirus (HPV) B19 induced aplastic crisis in a family leading to the diagnosis of hereditary spherocytosis (HS) is a very rare condition being barely reported in the literature. We herein report a 4-year-old girl, her brother, and their mother who all presented with progressive pallor and jaundice after a febrile illness. The HPV B19 was diagnosed using polymerase chain reaction (PCR) ...
متن کاملALLOGENIC BONE MARROW TRANSPLANTATION IN APLASTIC ANEMIA
Eighteen patients, twelve men and six women, with aplastic anemia underwent allogenic bone marrow transplantation (BMT) from HLA-matched siblings during the period of 1990 to 1996. The conditioning regimen was cyclophosphamide with or without busulfan, depending on the cause of aplasia. Antilymphocyte globulin (ALG) and cyclosporine were used for rejection and acute GVHD prophylaxis, respe...
